Keeping a completed copy of this form in your car (and it's also a good idea to tape one to your pet's travel crate) will help ensure your pet's best interests are looked after according to your wishes in the event of an accident if you are injured badly enough not to be able to specify care for your pet(s) who are travelling with you.</description>
